Each header should only include the other headers it needs to compile;
`devtools/reduce-includes.sh */*.h` does this. The C files then need
additional includes if they don't compile.
And remove the entirely useless wire/onion_wire.h, which only serves to include wire/onion_wiregen.h.

This appends the extra_tlvs to the internal wire htlcs "added" and
"existing" for the extra tlvs to be handed to lightningd.

This is such a simple struct that we can actually define it in csv.
This prevents us from accidentally breaking the ABI in future.
I tested this hadn't accidentally changed the wire format by disabling
version checks and using an old hsmd with the altered daemons and
running the test suite.

And turn "" includes into full-path (which makes it easier to put
config.h first, and finds some cases check-includes.sh missed
previously).
config.h sets _GNU_SOURCE which really needs to be done before any
'#includes': we mainly got away with it with glibc, but other platforms
like Alpine may have stricter requirements.

If we're over the dust limit, we fail it immediatey *after* commiting
it, but we need a way to signal this throughout the lifecycle, so we add
it to htlc_in struct and persist it through to the database.
If it's supposed to be failed, we fail after the commit cycle is
completed.

Note that it's channeld which calculates the shared secret, too. This
minimizes the work that lightningd has to do, at cost of passing this
through.
We also don't yet save the blinding field(s) to the database.

We currently abuse the added_htlc and failed_htlc messages to tell channeld
about existing htlcs when it restarts. It's clearer to have an explicit
'existing_htlc' type which contains all the information for this case.

Instead of making it ourselves, lightningd does it. Now we only have
two cases of failed htlcs: completely malformed (BADONION), and with
an already-wrapped onion reply to send.
This makes channeld's job much simpler.

tal_count() is used where there's a type, even if it's char or u8, and
tal_bytelen() is going to replace tal_len() for clarity: it's only needed
where a pointer is void.
We shim tal_bytelen() for now.

There are three cases:
1. failcode is 0, scid is NULL, failreason is the onion to fwd.
2. failcode is non-zero, but UPDATE bit not set. scid is NULL, failreason NULL.
3. failcode has UPDATE bit set. scid is non-NULL, failreason is NULL.
Assert these on marshaling, and only send the parts we need so unmarshal is
always canonical.

The master tells us the short_channel_id of the outgoing channel when
failing an HTLC, but channeld didn't store it anywhere. It also
didn't tell channeld the short_channel_id in the case where we're
reconnecting and it's feeding us an array of failed htlcs.

I'm not completely convinced that it's only ever set to a failcode
with the BADONION bit set, especially after the previous patches in
this series. Now that channeld can handle arbitrary failcodes passed
this way, simply rename it.
We add marshalling assertions that only one of failcode and failreason
is set, and we unmarshal an empty 'fail' to NULL (just the the
generated unmarshalling code does).
