Rusty Russell 5b9b056853 doc: introduce new state DUALOPEND_OPEN_COMMITTED. ...

This is the variant of DUALOPEND_OPEN_INIT which you see once
the channel is in the db: we'll be adding it next, but to reduce
clutter the docs are added as a separate commit.
