This removes the index array from code after increase_flows()m, so we use the flows
array directly.
The next step will be to make increase_flows() use the flows array, and remove the
index array indirection entirely.

Any file in this directory which is executable and whose name only
consists of alphanumeric characters, space, '.', '-' or '_' will be
automatically loaded when lightningd starts (unless suppressed with
commandline options).
